The Assistant Manager/Senior Executive (AM/SE) of Care Transition Services (CTS) will support the operations and managementof care transition services and programme between the NHG hospitals and the community. The staff should also provide an overview of all the engagement and programmes between the hospitals and community.
The job holder will support the satellite office in each hospital by working closely with the hospital specialists on right siting initiatives and to work with hospital operations to review discharge practices and work processes. This includes leading a team of Care Coordinators to facilitate right siting of discharge patients appropriately and efficiently into primary care and community care sectors.
The job holder will also support in seamless bi-lateral flow of care between hospital and community by supporting Clinical Liaisons in the hospital to support in the development of clinical pathways and operational processes for appropriate patient flow between the hospitals and community. The Executive will coordinate with other units of Population Health Campus to link up resources in the community and primary care sector to the hospitals.
