Essential Duties and Responsibilities:
The core duty of the Cancer Navigator is to provide care and support for adult cancer patients through psycho-social and emotional support which contribute to patient's overall well-being and improved quality of life.
- Provide support and assistance for the client depending on their needs (newly diagnosed, actively receiving cancer treatment, palliative/end-of-life, survivorship)
- Provide psycho-social and emotional support for clients and their caregivers
- Link clients with relevant and suitable resources and services (such as financial assistance, food rations, transportation, mobility equipment, medical consumables etc.)
- Follow up with community partners to ensure continuity of care after referrals
- Refer client for counselling, and spiritual or mental health support as and when the needs arise
- Provide information or coordinate for clients to receive psychoeducation from medical/healthcare practitioners which could help them have better understanding of the cancer diagnosis, and its treatments and side effects
- Maintain relationship and communicate frequently with clients and caregivers to assess their coping as they navigate their cancer journey
- Advocate for the needs of cancer patients and create awareness on cancer through community programs and publicity campaigns
- Document case recordings according to interventions provided for the client
- Facilitate support group session
- Carry out assigned administrative or coordinating duties
Education and/or Work Experience Requirements:
- Degree in Counselling, Psychology
- Demonstrated experience in case management and supporting people impacted by illnesses/multiple adversities
- Skills in demonstrating warmth and empathy to individuals impacted by illnesses/multiple adversities
- Interest in providing support for individuals/families from the lower socio-economic status
- Open to work with clients of various age groups (children, adolescents, young adults, middle adults, older adults)
Other Requirements:
- Able to coordinate logistical requirements for in-house and external programmes
- Able to work on weekends as required by supervisor if the need arises (for outings with clients, support group sessions, outreach programmes)
- Ability to travel for home-visits
