Care Coordinator (Advanced Care Planning) #HAL

Working Hours: Mon-Fri office Hours

Job Description:

- Engage all suitable referrals in Advance Care Planning discussion and facilitate the completion of Advance Care Plans for patients.

- Ensure effective communication of patients Advance Care Plans to health care professionals and other stakeholders.

- Follow up and review outcome of Advance Care Planning, including honouring of care and treatment preferences (where possible).

- Follow up on concerns raised during Advance Care Planning discussion, by providing feedback to the healthcare teams and other stakeholders.

- Work in partnership with stakeholders to raise awareness of Advance Care Planning.

- Conduct and participate in Advance Care Planning advocacy and facilitator training.

- Any other duties as assigned by the reporting officer.

Requirements

- Min Bachelor Degree in Nursing, Psychology, Social Work, Counselling or Allied Health relevant to the job.

- No experience required as training provided
