Care Coordinator (Public Healthcare) REF:HXP

Job Description:

• Support in Healthier SG related work
• Provide administrative support to the clinics when patients need referral to community and social partners, including day care and home care service etc.
• Liaise with the service providers in arranging medical appointment for patients
• Truck and submit monthly medical screening reports
• Work closely with the nurses to identify patients who require nurse counselling
• Perform right-sitting work, coordinating the transfer of patients from polyclinics to GP clinic where relevant
• Any ad hoc duties as per assigned

Requirement:

• Diploma in Healthcare Management/ Counselling/ Social Work/ Business Management or equivalent
