Job Description

You will assist the Medical Social Worker in assessing, evaluating, planning, coordinating resources and services for the continuity of care for the residents & collaborate with multidisciplinary health service professionals to develop care programs for timely treatment or recovery.

-Monitoring of Bed Occupancy Rate and meeting admission KPIs on a monthly basis

-Keeping track and managing the residents data and information

-Ensuring proper filing of residents data and information

-Applying financial schemes and monitoring the payouts for residents on a monthly basis

-Monitoring and ensuring that residents Household Means-Test is renewed before expiration

-Preparing Medifund reports and documents to present to Medifund Committee

-Answering and addressing queries and concerns from residents and their next-of-kin

-Any other case management work as assigned

Requirement

-Minimum Diploma in Nursing, Social Work or other related disciplines

- Minimum 0- 1 year experience in community healthcare settings in Singapore
