Job Description:

• Identify patients proactive screening daily
• Provide patients and their family education on disease process, causes of disease conditions, health education on risk factors, nutritional counselling, smoking cessation advice, exercise & coping strategies at home following discharge
• Screen, coordinate recruitment of patient into the outpatient programme
• Triage the referral, conduct patient for more information, ensure correct referral to appropriate exercise rehab program with sufficient time allocation for patient's visit
• Updates and maintains patient consultation/ CPR training statistics and CPR support group
• Any ad hoc duties as per assigned

Requirement:

• Bachelor Degree in Nursing or Allied Health or Science related.
• Minimum 3-5 years experience
