Case Management Officer (Cardiology) - Ref:HC

Job Scope:

• Reviews patients and assess their suitability for enrolment under Cardiac Rehabilitation Programme (CRP).
• Provides patient and their family information on disease process, causes of disease conditions, health education on risk factors, nutritional counselling, smoking cessation advice, exercise & coping strategies at home following discharge.
• Become point of contact for patient, family and multidisciplinary team.
• Coordinate and schedule Pre-Rehab Assessment clinic and Post Rehab Clinic for patients.
• Coordinate patients access and treatment sequence where required for the specialty.
• Input the information into the Rehab Clinic database.
• Conducts nurse-led specialty education session for patients and their families.
• Assist in purchase and work requisition processes.
• Ad hoc duties as assigned by the Head of Department.

Job Requirements:

• Minimum Degree, preferably in Nursing / Allied Health / Science or equivalent
• Excellent interpersonal and communication skills
• Preferably with working experience in a similar setting
• Proficient in using MS office
