RESPONSIBILITIES
- As a member of the CREST (Community Outreach Team), participate in outreach events with the team to raise public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals through screening and assessment.
- Triage cases referred to centre and conduct initial assessments and intake interviews with clients to determine their needs and suitability for enrolment into the programme.
- Provide case management services including assessment, psychosocial interventions (including basic emotional support and counselling), & monitoring of clients and their caregivers enrolled into the CREST programme, via home visits and/or phone-calls.
- Coordinate care and support to clients with or who are at risk of mental health issues and/or dementia with the appropriate resources.
- Provide service linkages and referrals to right site persons requiring mental health/dementia-related support services to the appropriate services and resources.
- Advocate for clients needs and assist in bridging gaps in service provision within the community
- Network and collaborate with government agencies (e.g., AIC), community partners (e.g., PA, Silver Generation Office, Family Service Centres, Active Ageing Centres), health and social care providers (GPs, hospitals, SSOs), to enhance support for persons with mental health conditions and dementia and their caregivers.
- Maintain accurate and up to date documentation of casework and maintain clinical/operational data for data submission and reporting, in accordance to programme requirements.
QUALIFICATIONS- Bachelor Degree in Counselling/Social Work/Psychology or related discipline.
- Preferably with at least 2 years work experience in social services field (MOH or MSF) performing case management tasks.
- Training in mental health/dementia care or experience working in a mental health/community care setting will be a plus.
- Able to work independently as well as be a team player.
- Possess good interpersonal and communication skills.
OTHER INFORMATIONWorking hours:
44 hours per week (normal office hours)
- 8.00 am to 5.15 pm (Monday to Friday, 45 minutes lunch break) or
- 8.30am to 5.45pm (Monday to Friday, 45 minutes lunch break)
- 8:00am to 12:30pm (Saturday, once a month)
