Job Responsibilities:
- Conduct initial consultation with participants using data gathered from a digital health platform
- Create and implement personalized health plans that are aligned with lifestyle goal(s) set by a General Practitioner (GP) utilizing digital tools and resources
- Use data and analytics to track participant progress and adjust plans as needed
- Monitor participant adherence to the program and encourage continued participation
- Provide ongoing support and guidance to participants through virtual coaching sessions and personalized nudges
- Communicate effectively with participants via phone and nudges
- Maintain accurate and confidential client records
- Consolidate and routinely update the list of existing activities available within the Queenstown community
- Stay up-to-date with the latest digital health technologies and trends
Job Requirements:
- Possess a diploma equivalent to professional qualifications in Nursing, Social Work, Allied Health, Health Services Management, Health Management & Promotion, etc. Those with Nitec in Nursing and relevant clinic work experience will be considered.
- 1 - 3 years of experience in healthcare and/or community health settings in Singapore is preferred.
- Strong team player, with a natural ability to interact with patients/residents, healthcare team, and community partners of all levels.
- Self-motivated and able to work independently and manage a high volume of clients
- Strong communication and interpersonal skills. The ability to use local languages and dialects will be an added advantage
- Strong understanding of health and wellness principles and ability to apply them in a virtual setting
- Willingness to learn and adapt to new technology
- Proficiency in MS office, including Word, Excel, Powerpoint and Outlook
- Motivation interviewing and/or Health Coaching certification is a plus
