Working Hours:

Mon to Thu, 8:30 am - 5:45 pm

Fri, 8:30 am - 5:30 pm

One Saturday/4 weeks, 8.30 am - 12.30 pm

Total of 42 hours per week

Job Scope:

• Perform social and financial assessment of patients and make recommendations.
• Provide counselling to patients and their families and lead in family conferences.
• Liaise with relevant organizations for collaboration and networking in care of the elderly.
• Attend to the social needs of destitute patients.
• Participate in a multi-disciplinary team on patient-care.
• Conduct groupwork with the elderly patients or their caregivers.
• Implement initiatives and support programs that enhances the care and support of elderly patients and their caregivers within the community.

Requirements:

• Advanced Diploma/Deg in Social Work
• Some experience in social work will be advantageous
