Job Description

  • Assist in administrative work in the delivery of advisory services to patients and their families.
  • Conduct assessment / home visits when necessary.
  • Liaise with relevant agencies / authority / patients family.
  • Assure that all referrals are attended to by relevant personnel.
  • Liaise with nurses on patient-related matters.
  • Assure that all referrals are attended to by self or relevant personnel.
  • Act as a channel of communication between management and the customer care coordinator.
  • Handle difficult customer and communicate escalated issues to the Manager.
  • Keep update records of patients ( electronic or written data)
  • Up-to-date the relevant information of government's policy with regards to the elderly care Services and funding.
