Job Overview:
- Role: Care Coordinator
- Location: Redhill
- Salary: Up to $3,000 for candidates with relevant diplomas and internships up to $3,300 for those with prior experience in Social Work Associate roles.
- Additional Benefits: AWS (Annual Wage Supplement) and VB (Variable Bonus).
Key Responsibilities:
- Support Clinical Team: Help clients apply for financial schemes, update renewal applications, and assist in administrative processes.
- Onboarding Process: Manage onboarding of new clients, including scheduling and assisting with subsidies and rehabilitation inquiries.
- Basic Case Management: Help assess client and caregiver needs, with guidance from case managers or social workers.
- Referral Assistance: Assist clients and caregivers in accessing appropriate community services and support.
- Collaboration: Work with multidisciplinary teams and assist with joint home visits and case discussions.
Qualifications:
- Minimum Requirements: Diploma in healthcare management, counseling, psychology, or related fields internship experience can be considered.
- Experience: Previous experience as a Social Worker Associate or in a similar role can warrant a higher salary offer.
