1. Job Functions
Client Assessment and Planning
- Conduct intake & needs assessments to identify clients needs, strengths, and challenges.
- Develop and implement service plans with measurable goals and objectives.
- Monitor and adjust service plans as needed to ensure clients achieve their goals.
Resource Coordination
- Connect clients with appropriate community resources.
- Collaborate with other service providers, agencies, and organizations to ensure comprehensive support for clients.
- Maintain an up-to-date directory of community resources and services.
Advocacy and Support
- Advocate for clients needs, including healthcare, education, and social services.
- Foster relationships with community partners and stakeholders to enhance service delivery.
Case Management and Documentation
- Maintain case files, including assessments, service plans, progress notes, and outcomes.
- Ensure all documentation complies with agency policies and relevant regulations.
- Prepare reports and statistics on caseloads and program outcomes as required.
- Manage Client Database.
Program Development & Volunteer Engagement
- Work closely with program team to develop and deliver services to clients.
- Engage and support volunteers to ensure sustainability of service delivery.
Assist with PERTAPIS events & projects where required.
2. Candidate Requirements
- Possess Diploma/Degree in Social Work or Psychology or Sociology or a related field.
- Preferably with Min. 2 years of relevant experience.
- Experience working with low-income families and with volunteers is a plus.
- As most clients are Malay speaking, bilingual in both English & Malay is advantageous.
- Outstanding verbal, written, and interpersonal communication skills.
- Strong problem-solving skills and attention to detail.
- Pro-active, a good team player and able to work independently.
- Ability to multi-task and meet deadlines with minimum supervision.

Monday - Friday, 9am to 6pm
