As the Head of Psycho-Social Care, you will provide visionary leadership in reshaping the care framework for persons with dementia and mild cognitive impairment (PMCI). You will drive the integration of evidence-based practices and develop sustainable, person-centred care models. This role involves working with multi-disciplinary teams, community partners, and caregivers to create an environment where dignity, wellness, and quality of life are at the forefront.
Key Responsibilities:
- Leadership & Strategy: Lead the strategic direction of the psycho-social care team, setting benchmarks and developing new care streams for PWD and PMCI in the community and the Lodge.
- Innovative Care Models: Synergize PCC with the Ability-Centered Care (ACC) model and mindfulness-based approaches, creating scalable, sustainable care solutions for evolving profiles of PWD and PMCI.
- Community Engagement: Engage and empower caregivers and community partners to co-create innovative care solutions, expanding services beyond Singapore through collaboration and knowledge exchange.
- Human Capital Development: Build and grow a high-performing, compassionate team that embodies AHL's values of respect, responsibility, and resilience.
- Quality Assurance: Develop robust systems for continuous improvement in care delivery and ensure compliance with health, safety, and regulatory requirements.
- New Business Streams: Lead efforts to create new, sustainable service models that meet the needs of diverse client profiles while ensuring affordability and scalability.

Requirements:

- Strong proficiency in strategic planning, project management, and evidence-based practice.
- Certification as a trainer or in technology-enabled care models is an added advantage.
