Job Scope

• Evaluate, plan and implement care of patients with chronic disease.
• Conduct teaching and counseling for patients and their families / caregivers.
• Coordinate patient care where required.
• Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services.
• Ensure database is relevant and up to date.
• Assist to review and develop patient education materials
• Review and evaluate work processes
• Train and guide newly joined coordinators in delivery of patient care where needed.
• Participate in quality improvement activities.

Requirement

• Bachelor's Degree in Public Health / Life Sciences or equivalent
