Reporting to the Nurse Manager (Home Care), you will provide patient-centered and compassionate care tailored to the needs and conditions of home care patients, while adhering to organizational standards, legal requirements, and stakeholder expectations. You will also play a key role in orienting, teaching, and supervising new and junior staff, fostering a skilled and effective nursing team.
Key Responsibilities:
- Ensure nursing care services are safe, competent, and personalized. Maintain high standards in documentation, treatment, patient monitoring, and infection control.
- Manage complex patient care while applying clinical skills, knowledge, and critical thinking in care delivery.
- Continuously monitor, reassess, and evaluate patients conditions, implement care plans, and support families in managing advanced disease.
- Coordinate patient care in collaboration with advising physicians, provide direct care, evaluate conditions, and serve as a liaison between families and healthcare providers.
- Establish rapport with patients and their families or caregivers, assessing home needs for training and necessary equipment to enhance patient living conditions.
- Allocate equipment as needed to ensure adequate resources for care delivery.
- Follow up on patient care to maintain continuity and adherence to treatment plans.
- Administer nursing care tasks such as medication administration and wound dressing as required.
- Maintain accurate documentation of patient conditions and compile data on referrals, inquiries, and attendances.
- Participate in and facilitate Multidisciplinary Team meetings, collaborating with other teams to ensure seamless patient care.
- Implement active safety measures to protect patients.
- Comply with organizational and legal standards regarding the storage and management of drugs and medical supplies.
- Uphold confidentiality and discretion in all patient care matters.
- Ensure proper use and maintenance of equipment and manage the economical use of supplies.
- Address public inquiries professionally and appropriately.
- Adhere to the policies and procedures of AH and ensure compliance with the Singapore Nursing Board's Standards of Practice for Nurses and Midwives, along with the Code of Ethics and Professional Conduct.
- Stay informed of updated industry guidelines and continuously develop new skills relevant to the role.
- Maintain high standards of cleanliness and organization in the workplace.
Requirements:
- Diploma in Nursing from a recognized local Polytechnic, registered with the Singapore Nursing Board. Experience in a hospital or hospice setting is preferred.
- Proficiency in IT and relevant systems.
