Job Responsibilities- Assess referrals and co-ordinate to process placement applications in a timely manner.
Conduct means testing, social and financial assessments of residents and their families.- Provide financial counselling to residents and their families, assist with financial applications where necessary.
Work with the multi-disciplinary team on holistic care management of residents.
- To have proper report and documentation of all cases of LKHSC.
To upkeep the confidentiality of the cases seen and adhere compliance to PDPA.- To maintain records to facilitate submission of the periodic returns and ad hoc reports for accountability purposes and as per request.
To liaise with the various government agencies, Agency for Integrated Care, restructured hospitals, and other social service / healthcare organisations with regards to cases, transport co-ordinations, and medical appointments.
- To undertake full spectrum of case management from application, intake assessment interviews, admissions to follow-up with discharged residents into the community.
To process applications and request for subsidies, means testing and the various financial assistance schemes.- To attend to public enquires on nursing home services and provide relevant information and referral to them.
To undertake any other ad hoc duties as assigned by the Management and/or Director of Nursing.
- To ensure the psycho-social and emotional well-being of the residents are well taken care of.
To write/review/update of Medical Social Work Department's Standard Operating Procedures (SOP).- To review cases and make recommendations to:
- LKHSC Management for cases regarding subsidy deviations
- LKHSC Management regarding cases that are seeking admission into LKHSC
- LKHSC Medifund Committee regarding cases that are applying for Medifund assistance
Degree in Social Work with 5 years experience as MSW- Good written and spoken English
Good communication, interpersonal skills and independent
- Casework, counselling, conduct social and financial assessment and make recommendations, attend court and draw up care plans
Provide support to residents and family members- Work closely with multidisciplinary professionals and organise care plans for residents. Liaison with relevant govt agencies
