Who Are We:
Stroke Support Station (S3) is a non-profit organization in Singapore dedicated to enhancing the quality of life for stroke survivors and their caregivers. S3 provides a range of programs focused on rehabilitation, empowerment, and community reintegration. Through specialized activities, wellness programs, and caregiver support, S3 aims to foster physical, emotional, and social well-being. The organization also advocates for stroke awareness and collaborates with healthcare professionals, volunteers, and partners to offer holistic, person-centered care for the stroke recovery journey.
You Will:
- Conduct accurate and holistic occupational therapy specific assessment for clients during admission, periodic care plan reviews and discharge
- Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
- Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
- Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
- Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
- Recommend appropriate assistive device and equipment as needed
- Formulate discharge and continuity of care plans based on clients performance in consultation with relevant stakeholders
- Facilitate community integration through collaboration with other community partners as required
- Mentor and supervise therapists and support staff for professional development and day-to-day job scope
- Identify safety risks and issues, recommend preventive and corrective care measures
- Conduct service and clinical audits, and review audit findings
- Conduct in-house training for staff, caregivers and volunteers
- Engage in continuing education to further skills and knowledge in the field of practice
You Are:
- Someone who have a Degree or Diploma in Occupational Therapy and is Fully registered with AHPC
- Able to converse in English and other languages due to the diverse profiles of our clients
- Equipped with at least 5 years experience as an Occupational Therapist, preferably with 2 years in Neuro-rehab or community care setting
