Responsibilities:
- Conduct initial assessment, follow-up reviews and other specific assessments of designated patients in day rehab and day care.
- Develop individual care plans based on patients needs, adopting a multi-disciplinary team-based approach and implement appropriate interventions / activities.
- Review and re-assess individual care plans regularly and/or when there is a change in patients condition for both day rehab and day care clients.
- Develop and manage therapy programmes and activities for centre
- Conduct assessments for clients during admission and periodic reviews.
- Oversee and conduct therapy programmes involving technologies.
- Perform any other duties as assigned.
Job Requirements
- Bachelor degree in Physiotherapy
- At least 2 years of working experience in physiotherapy
- Experience in Senior care/ Day care/ Wellness Centre
- Full registration with Allied Health Professions Council
