Primary Healthcare Group Provider is inviting keen individual to join its team as Primary Care Coordinator
Job Responsibilities
- Provide general support in (GP) clinics and work closely with GPs
- Liaise with the service providers in arranging physical screening appointments with patients
- Provide administrative support to GPs when patients need referral to community and social partners for certain services e.g. day care, home care services etc.
- Track and submit monthly reports
- Provide technical support to elderly or less tech-savvy patients in using related mobile applications
- Work closely with the nurse to identify patients who might require counselling
- Support patients in adhering to their care plans by providing simple lifestyle and behavioral modification advice
- Support the admin team in administering surveys and collating data for analysis and reporting
- Provide support in gathering feedback on ground issues
Job Requirements
- Diploma holders with experience in frontline service
- Prefer work experience related to healthcare or social/community services industry
- Proficient using MS Excel
- Positive attitude and willingness to learn
- Service-oriented and patient-focused
- Able to work independently with minimal supervision
