We are looking for a forward-thinking Principal Clinical Pharmacist to play a key role in revolutionising patient care within our Hospital-at-Home program. This flexible role, available as full or part-time, allows you to practice at the top of your license, leveraging your expertise in pharmaceutical care and medication management to make an impact on patient outcomes.
Key Responsibilities:
- Attend daily interdisciplinary rounds to collaborate closely with physicians, nurses, and other healthcare professionals to develop and implement patient care plans in a home-based hospital setting
- Conduct daily virtual ward rounds with the patients, driving innovation in pharmacological care and ensuring optimal patient outcomes.
- Utilize your extensive knowledge of inpatient care to manage complex cases across a wide range of specialties
- Innovate and optimize medication regimens, enhancing patient adherence and minimising adverse effects, while contributing to the transformation of clinical care models
- Provide expert consultation on medication management, ensuring the highest standards of safety and efficacy
- Engage in patient education, empowering patients with the knowledge they need to manage their therapies effectively at home
- Contribute to clinical governance, strategic planning, and the development and implementation of protocols and guidelines that will shape the future of hospital-at-home care at NUHS
- Embrace continuous learning and professional development opportunities, positioning yourself at the forefront of healthcare innovation
Clinical Practice also includes the following pharmacy duties: - Provide general clinical pharmacy services to promote wellness and ensure optimization of medication therapy outcomes for patients, including ensuring accurate medication records, medication reconciliation, and ensuring medication adherence
- Identify and define patient-specific medical problems, recommend, implement and manage patient pharmacotherapy, review, counsel, dispense and/or supply medications as needed
- Provide ongoing monitoring and adjustment of patient's drug therapy, resolve and/or prevent patient's drug-related problems in accordance with departmental policies and procedures, work and/or consult with other health care providers to improve patient outcomes
- Promote and monitor for adherence to prescribing guidelines or drug use protocols.
- Provide professional advice and evidence-based drug information to patients and the health care team
- Work with patients, caregivers and other health care providers to ensure safe, cost-effective, timely distribution and quality use of medication that will best meet the patient's needs, achieve desired patient outcomes and assure continuity of services
- Communicate with patients and/or caregivers on a group or individual basis to ensure understanding of prescribed drugs, to resolve or prevent drug- related problems and to assure optimal patient cooperation in drug therapy
- Document clinical activities as defined in the competency development framework and/or department guidelines
- Lead and/or serve on departmental and/or institutional clinical practice committees and/or other relevant committees as assigned
- Education
- Identify, plan, implement and provide educational programs and training to entry level health care professionals and the public
- Plan and contribute to the development or publication of high quality patient education materials and articles targeted at general professional education
- Lead the development and implementation of pharmacy professional standards, competency framework and/or assessments for future clinical pharmacists for this ACP telehealth service
- Lead and contribute to the continuing professional development strategy
- Research
- Lead, initiate and participate in clinical research independently or in collaboration with other health care disciplines
- Publish scientific papers in peer reviewed journals and/or present findings at key local or overseas scientific meetings
Secondary Duties (10%)
- Lead and contribute to the review and evaluation of services, policies and processes, and identify and implement medication safety and quality improvements initiatives to ensure optimal outcomes under NUHS@Home
- Participates in and/or undertakes responsibility for projects and activities which supports the goals of the department and organisation at department or institution level
- Maintain and up-to-date professional portfolio
- Any other duties or responsibilities as assigned
Requirements
- Possess MSc (Clinical Pharmacy) or Pharm D or Board Certification
- Completion of a Collaborative Prescribing Course within last 2 years
- A minimum of 5 years of experience in clinical experience (ideally in inpatient care)
- Registered with the Singapore Pharmacy Council
- Possess extensive and current knowledge of pharmaceuticals, clinical practice guidelines and protocols
- Possess strong leadership qualities
- Possess good supervisory, negotiation and managerial skills
- Excellent clinical pharmacy and evidence-based medicine knowledge and skills
- Excellent clinical problem solving, judgment and decision making skills
- Excellent knowledge of drug information research tools, clinical research methodologies and outcomes evaluation
- Excellent listening and communication skills, with the ability to educate and engage patients, and work collaboratively with a diverse healthcare team. Ability to speak in a second language in addition to English will be helpful.
