Responsibilities:
Program Management:
- Develop, implement, and oversee CoC programs and services.
- Monitor and evaluate program performance to ensure objectives are met.
Collaboration:
- Foster partnerships with healthcare providers, social service agencies, and community organizations.
- Facilitate communication and coordination among stakeholders to enhance service delivery.
Data Management:
- Maintain accurate records and data related to program activities and outcomes.
- Use data to inform program improvements and ensure compliance with quarterly reporting requirements.
Outreach and Engagement:
- Conduct outreach activities to promote CoC programs within the community.
- Engage community members and stakeholders to understand their needs and gather feedback.
Compliance and Quality Assurance:
- Ensure all programs comply with relevant regulations and organizational standards.
- Implement quality assurance processes to maintain high standards of service delivery.
Requirements:
- Bachelor's degree in Social Work, Public Health, Healthcare Administration, or a related field.
- Advanced degree or relevant certifications (e.g., Project Management Professional) are advantageous.
- Minimum of 3-5 years of experience in program coordination or management within healthcare or social services.
- Experience with community-based care programs is highly desirable.
- Strong organizational and project management skills.
- Excellent communication and interpersonal abilities.
- Ability to work collaboratively with diverse groups.
- Knowledge of regulatory requirements related
