Conduct group activities for carers and/or persons with dementia.
Engage with various stakeholders to ensure the smooth running of programme.
Support outreach events and initiatives.
Explore new avenues of collaboration with community partners and stakeholders.
Provide case management support to carers of persons with dementia.
Ensure proper documentation of client follow-ups.
Update statistical data in a timely manner for funder reporting.
Perform other duties as assigned.
Requirements
Degree in Psychology, Social Work, Community Development, or a relevant discipline, with a minimum of 2 years of experience in community engagement and/or case management.
Prior experience working with persons with dementia or caregivers is an added advantage.
