About The Role
The Youth Integrated Team (YIT) focuses on youths at-risk of or with mental health conditions, aged 12 to 25 years, by conducting targeted outreach to increase awareness of mental wellness, provides assessment, therapeutic intervention and case management while engaging youth for ongoing monitoring and support.
The staff is responsible for providing programme and practice leadership for the YIT team.
Key responsibilities include
. Conduct assessment and provide intervention, including case management and counselling, caregiver training, and support to youths with mental health issues
. Plan, implement and review services and activities to meet the needs of the youths and/or caregivers.
. Provide case supervision, lead in training needs analysis and recommendations, and performing in-house training
. Lead the discussions and review on practice protocols, processes, and guidelines
. Work with community partners to setup ecosystem of care to facilitate better service navigation, help seeking experience of youths and their caregivers, service integration and wrap around care
. Analyse and review data to identify trends, gaps and lead programme evaluation, service review and research discussions
. Establish volunteers partnerships to optimize resources and impact
. Lead in key stakeholders engagement for service development
Candidate Requirements
Technical Competencies:
. Degree / Graduate Diploma Masters in Social Work
. Strong practice leadership, experience in providing supervision
Programme leadership experience
. At least 5 years experience in social services
. Must be registered with respective professional associations and qualify as approved supervisor
Core Competencies:
. Enjoys working with youths and mental health concerns
. Interest in pioneering work
. A team player and equally adept at working independently
. Emotional resilience
