What youll do as a Senior Social Work Assistant!
We are looking for a compassionate individual to connect with adolescent patients and their caregivers. In this role, you will administer structured mental health assessment tools, perform basic biopsychosocial evaluations, and triage / refer the patients to the appropriate multidisciplinary team members. Additionally, you will provide psychoeducation and para-counseling to the patients and their caregivers.
Your responsibilities will also include initiating outreach to adolescent patients visiting the clinic and accompanying the Medical Social Worker on field visits when required. Administrative duties are also a key part of this role, including, managing email and phone inquiries related to the adolescent mental health program, maintaining data and scheduling meetings..
You may be required to support the Medical Social Worker with case management tasks for other patient profiles and projects when required.

Preferred Qualification!
Minimum 4 years of relevant working experience in working with adolescents will be an advantage
Higher National ITE Certificate
Certificate/Diploma in community and social services or equivalent will be preferred
