Job Summary: The Renal Coordinator is responsible in educating mild chronic renal failure patients and end stage renal failure patients in self-care and teaching patients at risk of developing renal failure on retardation or prevention of the disease.

Responsibilities:

• Providing patient education for chronic kidney disease patients
• Presenting treatment options for moderate to end stage renal failure patients
• Providing financial counselling for new dialysis patients or patients who are going to initiate dialysis
• Providing counselling service
• Patient case management
• Coordinating of dialysis plan
• Monitoring of Ambulatory Blood Pressure
• Maintaining and updating of CKD (Chronic Kidney Disease) and ESRD (End Stage Renal Disease) Registry

Qualifications:

• Higher Nitec/Diploma or Degree in Nursing, Business or a related field
• Preferably 2 years experience in Healthcare industry
• Proficient in Microsoft Office applications
• Demonstrate strong communication and analysis skills
