You are required to ensure high standards of child-centric clinical and community practice within a Trans-disciplinary framework. You will work in a multi-disciplinary outpatient setting and provide supervision, coaching and social work input to a multi-disciplinary team.
You are also required to provide direct or indirect socio-emotional support, assessment and referral for financial assistance to mothers and child from early conception to first few years of the child through clinic appointments, home visits and community based activities. Where appropriate, risk assessments would also be required.
In addition, you will also be involved in the following duties.
- Maintain overview of team's cases.
- Triage cases for presentation.
- Ensure case documentations are promptly completed and filed.
- Conduct intake interviews and risk assessments.
- Assignment of cases to respective Keyworkers for service delivery
- Assess clients for needs and facilitate referrals to community partners or services for further follow-up.
- Conduct home visits
Requirements
- Degree in Social Work or Post graduate Degree in Social work
- At least 3-5 Year(s) of direct social work experience practice (especially in working with families with young children)
- A fast and efficient worker with the ability to multi-task
- Registered Social worker with the Social Work Accreditation Board (SWAB)
- Supervisory experience is preferred
