Job Summary
The Senior Social Work Care Manager is responsible in the implementation of care plans for clients to meet the Foundation's mission and operational targets and providing interventions enabling the older person to live in optimal health.
The role holder will assist the Head of Programme and Clinical Head from the Care Management Service department to carry out administrative work in the delivery of social services to our senior clients and caregivers. He/she shall utilise the system for processing information and statistics, conduct home visits and render assistance in financial assessment procedures.
Key Responsibilities
- To be responsible as the assessor, facilitator, and advocate for the clients interest, to meet operational targets of the Foundation and the funding authorities.
- To be responsible for the social work assessment and care planning.
- Work with the interdisciplinary team input to strategize and implementation care management interventions to meet the team and client care needs. Update and evaluate the progress in timely manner before discharging clients to the longer-term stakeholders in the community as per funder's requirement.
- To initiates and participates in quality improvement and evidence-based projects and implements quality improvement activities within their own area of work. The staff also contributes to the planning of client/ caregiver education, groupwork and programmes.
- To provides supervision to ACM, interns and contributes to continuing professional development activities for the ACM or peers.
Key Requirements
- Post Graduate Diplomq in Social Work
- Degree in Social Work or other relevant disciplines
- State registered social worker, registered with the Singapore Association with Social Worker is preferred
- Has at least minimum 4-5 years working experience.
- Professional certification or diploma in counselling is preferred
- Courses on care management is preferred.
Relevant Experience
- Working experience and enjoys working with older people.
- Work as case management or care coordination will be added advantage.
- A good team player, positive and have empathic skills
- Possive Good IT knowledge and skills
- Penchant for working with the elderly and conducting home visits.
- Fluent in English, Mandarin and 1 local dialect as ther role is require to communicate varies languages with senior person.
