SAMH manages the Mobile Support Team (MST), it is a community based allied health-led team which provides psycho-social therapeutic interventions for clients with mental health needs and support for their caregivers to maintain clients wellbeing in the community.
Purpose
Responsible for the provision of case management services and support for persons in recovery from mental health conditions/possible mental health conditions, and their caregivers.
Core Duties:
- Case management, implement and coordinate follow-ups with case plans
- Conduct home visits to clients and caregivers where necessary
- Provide individual and family counselling, group work and support
- Administrative support, to write case recordings, case reports and correspondence to agencies
- Collect and verify client-related information and compile updated records for reporting
- Outreach involvement, to conduct in-house and external talks, man information booths to promote SAMH services and to raise awareness of mental health concerns
- To collaborate with and support other SAMH programmes or projects when required
- Provide guidance for case management practices and facilitate the development of staff's growth with agency
- Other administrative and support duties assigned
Qualifications
- At least a recognized Basic/Honours Degree in Social Work, Counselling or Psychology
- 3 to 5 years of direct case management intervention experience in social service/mental health setting, preferably in social or counselling work
- Experience in supervisory role in the area of clinical management of persons with mental health conditions will be preferred
- Effective communication and strong interpersonal skills
- Able to communicate in English and Mandarin with the clients
