Job Purpose
- To provide counselling and psychosocial support for patients and families through case management, in-depth assessment of family dynamics, linking up with various social service agencies to facilitate care and discharge planning.
- To contribute to the community via supporting colleagues in needs assessment, program development, implementation and/or evaluation of programs.
Responsibilities
- Clinical and Case Management:
- Engage and build professional relationship with clients and significant others.
- Gather relevant information through intake assessment to determine needs and evaluate psychosocial status of patients and families.
- Develop, implement and follow up with appropriate care plans for patients and families.
- Provide illness-related psychoeducation, counsel and support patients/families experiencing psychosocial issues associated with the illness.
- Manage cases referred by Day Rehab Centre for care planning schemes application and psycho-education.
- Manage cases referred by Home Healthcare Team for Financial Assistance (Medifund/PWF/MT Deviation) and counselling support etc.
- Conduct home visits when appropriate and coordinate home modifications for needy patients.
- Establish and maintain working relationship with multidisciplinary team through regular conferences.
- Network and liaise with service providers, agencies to match patients to appropriate community services to ensure continuity of care.
- Assist in data collection and support action-based research.
- Adhere to protocols in accordance with clinical practice guidelines and integrated care pathways.
2. Professional Development:
- Engage in continual learning through regular case discussions, supervision and training (internal and external).
- Engage in research studies and quality improvement projects.
3. Programme Development:
- Assist and participate in the needs assessment, planning, delivery, monitoring and evaluating of programs when appropriate.
- Support psycho-educational groups for patients and/or caregivers.
4. Administrative:
- Ensure proper documentation of case files, referrals and other related documents for case management and audit purposes.
- Collect and submit regular statistics.
- Carry out relevant administrative duties as assigned by Supervisor.
Requirements
. Bachelor's Degree in Psychology / Counselling.
. Fresh graduates are welcome
