The Social Work Associate is responsible for conducting intake, information and referral services. He /she would also conduct engagement and preventive activities to the clients and/or caregivers in an individual or group-based setting.
He/She will be working closely with the Social Worker (Team Lead) and Social Worker/Case Worker in the planning and implementation of these activities, as well as to monitor clients condition and progress.
He/She also provides the daily administrative and operational support to the team, for the purpose of CREST service development and implementation.
He/She organises the logistics &/or resources to ensure the smooth running of CREST services. The Social Work Associate may also be involved in coordinating outreach events and activities, where required.
A. Programme Implementation
i. Conduct intake and needs assessment during first contact, prepare for necessary intervention that includes information and referral and basic counselling services to help
ii. Provide support to clients and/or caregivers through engaging them in meaningful activities such as cognitive stimulation activities or structured activities e.g. reminiscence, memory, linguistic and other appropriate activities) based on clients interest.
B. Administration and Staff Development
i. Provide administrative or logistical support to the team in running client-related support and/or other CREST activities.
ii. Assist in coordinating and planning outreach and engagement events with other community partners and agencies.
iii. In-charge of timely data collation and quarterly report submission, as well as generate reports, in accordance with the CREST programme. iv. Store and maintain data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
v. Coordinate the team training schedule, in consultation with the Social Worker (Team Lead).
vi. Ensure attendance in training and supervision sessions, where relevant, as part of the staff competency required by the CREST programme.
C. Other Duties
i. Prepare the necessary resource material to be provided to clients and caregivers. ii. Any other duties, as assigned.
Technical Competencies:
Ability to engage service users and community partners.
Able to use email, calendar, shared drives as well as common software and smart phone apps.
Core Competencies:
Resilient, passionate to serve the community and a team player.
Preferred achievements / characteristics:-
Qualification background: - Diploma in Social Services - 1-2 years relevant working experience - Fresh graduate is welcome to apply
Preferred:- Experience in Social Service Sector or Mental Health Sector
