Social Work Associate #HLY

Job Description:

• Participate in social work practice across disciplines
• Support in building rapport and professional relationships with clients and significant other
• Support needs and intake assessments such as conducting Means Testing, financial assessments, conducting home visits, etc.
• Provide support to Social Workers on case and care planning and interventions
• Prepare case notes and documents
• Support the evaluation of group work intervention processes and outcomes
• Perform community development work
• Support conduct environmental scans and community needs assessments

Job Requirements:

• Diploma in Social Sciences (Social Work) or Social Services
