Key Position Objectives:
Assist the Senior Social Worker under the social work team for supporting the residents/clients at Red Cross Home for the Disabled (RCHD) and Red Cross Home Day Activity Centre (DAC).
Job Scope:
- Responsible for conducting/renewing means tests and case management.
- Conduct initial screening and intake assessment for new referrals and manage care plans for the residents.
- Attending monthly case conference and multi-disciplinary meetings.
- Develop individual care plans and carry out regular review of the cases.
- Communicate and work closely with family members or caregivers for the residents basic needs, care plan, medical bills, etc.
- Liaise with hospitals, government agencies and other community partners relating to the residents care plans.
- Handle assigned administrative duties & reports.
- Attending to phone calls, video calls, and visitations.
- Carry out any other duties as assigned by the supervisor.
Technical Competencies:
- Good listening and communication skills
- Basic case management skills
- Familiar in using Google Suite
Core Competencies:
- Minimum Diploma in Social Sciences (Social Work) or WSQ Diploma in Community and Social Services
- At least 2 years working experience in social service sector
