Job Description
About the Role
You will be part of our community outreach team, also known as Community Resource, Engagement & Support Team (CREST), focusing on raising public awareness of mental health conditions and dementia, early recognition of at-risk individuals, providing emotional support to individuals and their caregivers and link individuals to relevant health and social care services where necessary.
Provide case management services from assessment, case management, working with systems, case conferences, and closure and to maintain proper and up to date case documentation
Plan, coordinate, execute and evaluate on-going programmes and services, including outreach, on-site/online public education
Conduct analysis, develop and implement mental health programmes and services to meet the needs and trends of the community
Attend to enquiries and provide information and referral services
Engage key community stakeholders to coordinate and implement mental health programmes
Coordinate with other professionals and mobilise community resources for service delivery
To provide supervision to junior social workers/caseworkers
Upkeep programme data, records, and documentation
Prepare and submit periodic reports for key stakeholders
Volunteer management and providing supervision for volunteers and interns
Opportunities to be involved in organizational level projects
Qualifications
Bachelor/Graduate Diploma in Social Work or relevant fields (Psychology, Counselling, Community Work)
Requirements:
Passion to work with seniors and caregivers
Knowledge and experience in mental health sector will be advantageous
High proficiency in written and spoken English with good facilitation and presentation skills; able to speak in mandarin and/or dialects would be essential for engagement with seniors
Strong interpersonal and communication abilities with individuals and systems at multi-levels
Good analytical and assessment skills
Team player with an ability to function effectively in a fast-paced environment
Strong organization skills and structured.
