Responsibilities:
- Review applications from AIC and interview clients and caregivers during the enrollment process for our programme or home. Conduct biopsychosocial assessments to evaluate the strengths, resilience, and support systems of clients and their families.
- Build rapport with residents/clients and their families. Maintain consistent communication with clients, addressing their concerns and goals, while promoting a positive outlook. Educate family members on clients physical and psychosocial needs, as well as ways to access internal and external resources.
- Provide case management for clients, coordinating with external agencies such as hospitals and polyclinics for those requiring financial assistance for medical appointments.
- Develop and implement Individual Care Plans (ICPs) for clients, offering therapeutic support as needed, and collaborating with other multidisciplinary team (MDT) members.
- Execute ICPs and monitor progress. Plan and implement programmes and activities aligned with SASCO values to meet client care needs.
- Coordinate with physical therapy (PT) and occupational therapy (OT) departments to help clients feel comfortable and confident.
- Submit 6-month ICP review reports and monthly client review records.
- Evaluate new, existing, and prospective clients based on their needs, preferences, and limitations.
- Update client particulars and social reports/reviews. Conduct casework evaluations and maintain case notes and documentation.
- Provide person-centered counseling to clients and families, addressing issues such as mental health concerns (e.g., depression, anxiety), coping with life changes, and adjustment difficulties.
- Conduct risk assessments for self-harm (e.g., suicide) and harm to others (e.g., family violence, elder abuse).
- Perform financial assessments and fund management, preparing periodic reports as required. Identify and refer cases for financial assistance, such as generating Means Test Subsidy Change Rate Reports from MOH for invoicing.
- Carry out any additional tasks as assigned by management.
Requirements:
- Degree in Social Work
- Passion/ Interest to work with the elderly in a healthcare setting
- 1 - 3 year of relevant experience
