Social Worker (1 year contract)
- Salary : $3,500 - $4,500
- Working days: Monday - Friday
- Working hours: 9am - 6pm
- Location: Eunos
Job Responsibilities
- Assess needs of seniors with care/service needs and formulate care plans for seniors
- Assist in the development and implementation of programmes for seniors through data
- Collection and the conduct of basic analysis
- Coordinate care for the seniors with relevant stakeholders
- Documentation of case notes
- Facilitate timely and appropriate service referral and provision to seniors
- Participate in case discussions with external service providers
- Provide case management services
- Perform needs assessment of seniors living in the community
- Prepare data for submission to relevant authorities
Job Requirements
- Degree in Social Work
- With relevant experience in Social Service Sector or related field is an advantage
- Training provided
