Job Summary
Purpose of job
Case management and psychosocial intervention for various clientel groups from youths, adults, caregivers, seniors with mental health challenges in the community in accordance to ethical and legal guidelines.
Key Responsibilities Areas
- Ability to provide case management, psychosocial intervention, care coordination and recovery support for clients with alleged or diagnosed mental health issues.
- Able to work independently to conceptualise, implement intervention plans and follow-up with clients.
- Support intake and referral services.
- Work together with community partners or stakeholders for case conferences pertaining to complex cases.
- Create programme, conduct and facilitate groupwork for clients and caregivers if needed.
- Provide talks to raise awareness of mental health issues and dementia.
- Update of sessions into the system for reports according to service requirement.
- For Senior Social Worker position, clinical supervision is expected.
- To undertake any duties as assigned by the Head, Community Mental Health and the Executive Director.
Requirements
Experience:
- Experience and/or knowledge in working with clients on Mental Health challenges and Person-Centred Care and recovery will be an advantage.
- Experience in case management, group work and community work will be an advantage.
- Good organisational skills, highly motivated and service mindset to serve the community.
- Ability to work independently and as a strong team player.
- Good administrative and computer skills with proficiency in MS Office
Qualifications:
Grad Diploma/ Degree in Social Work
Others:
Required some travelling and home visits to different locations.
