Staff Nurse (Homecare) - No experience required!

• assess, plan, implement and evaluate care for palliative patients in conjunction with the palliative and primary team
• prepare patients for doctor's consult
• support doctor in providing patient care
• attend to enquiries from of patients
• collaborate with home care team and healthcare professionals in the delivery of palliative home care to patients
• help patients to transit from inpatient setting to home and to continue with the care for them thereafter
• update home care team on patients conditions and follow up
• to ensure timely documentation and record keeping of nursing duties carried out.
