Roles and Responsibilities
- Assist the nursing team in providing personalised, specialised and cost-effective care.
- Monitor and report patients conditions and progress to senior members of the nursing team.
- Notify senior members of the nursing team of all unusual observations and/or happenings.
- Assist in the execution of discharge plans of patient care according to the assessed needs, as well as assist in the post-discharge care coordination of patients.
- Follow up on phone reviews and home and/or agency visits to ensure that services required in the community are in place.
- Perform home visits and assist to educate patients/caregivers on self-care management in the community.
- Assist senior nurses on follow-up phone reviews and home/agency visitations to ensure that services required in the community are in place.
- Educate patients and family members on the patients health conditions and help them gain a strong understanding of holistic care as necessary.
- Arrange appointments with client and or caregivers for new referral home visit.
- Conduct and document nursing assessment as appropriate.
- Raise individual care plan based on identified assessment needs.
- Provide nursing services as required, e.g., change of nasogastric feeding tube, urinary catheter change, dressing change, stoma care, monitoring medication compliance and medication packing, parameters/peripheral blood glucose monitoring, etc.
- Ensure that safety and infection control precautionary measures are practiced during home visits.
- Issue receipt and receive payment from client/caregiver at the end of each visit.
- Hand over cash payment collections to Finance department after home visit.
- Conduct the first assessment as appropriate to identify required needs, e.g., referral for Senior Mobility Fund (SMF), Community Services or Financial Assistance.
- Orientate and coach new nurses and serve as liaison, resource person and preceptor to nurses and trainees.
- Supervise the work of Staff Nurses and Enrolled Nurses.
- Participate and share knowledge and skills in peer education.
- Safety checks on all equipment used for home care.
- Contribute to administrative work as directed by Nurse Manager/ Nurse Clinician/ Assistant Nurse Clinician.
- Accept additional responsibilities as directed by Nurse Manager/ Nurse Clinician/ Assistant Nurse Clinician.
- Assist Nurse Manager/ Nurse Clinician/ Assistant Nurse Clinician to handle complaints and initiate service recovery.
- Assist Nurse Manager/ Nurse Clinician/ Assistant Nurse Clinician to audit and or appraise the performance of new and or junior nurses.
- Assist to supervise junior nurses as and when required.
- Initiate and maintain professional growth through Continuing Professional Education (CPE).
