As a transplant coordinator, you are able to impact the lives of our patients and their families. You are required to assess, educate and co-ordinate the care of patients and donor throughout transplant process for pre-intra and post-Haematopoietic Cell Therapy and Transplant (HCTT) in KK Children's and Women's Hospital.
You are required to communicate all aspects of patient pre-HCTT care plan with other multi-disciplinary team. Providing support to co-ordinate patient admission and outpatient scheduling and provide counselling and coordinate Human Leukocyte Antigen (HLA) for patients, siblings and/or family members.
You will be assisting to follow up on the results with clinical transplant manager and transplant physician in-charge.
You will be required to attend and make records proceedings of regular meetings, family conferences and working closely with all internal and external stakeholders. To assist with co-ordination of HCTT-related events when necessary.
You will be maintaining proper documentation and filing of records. Clinical entry of information for pre- and post-transplant and to comply with local and international requirements for accredited transplant centres. You will be required to prepare monthly transplant records and data.
You will also be providing guidance and education to fellow colleagues, junior nurses and nursing students in the field of HCTT-related matters.
Job Requirements
- Degree in Nursing or Life/Health Sciences
- 3-5 years clinical nursing experience in an acute hospital setting or with 1-2 years of specific experience in care of HCTT patients.
- Excellent interpersonal skills in dealing with delicate and sensitive situations
- Pleasant disposition and ability to work independently
- Able to multi-task and perform time-sensitive work in collaboration with a multidisciplinary team
- Good team player and ability to work effective with multiple stakeholders
